The worst ever oil platform tragedy took the lives of 167 men and revealed a fragile safety methodology aboard the most productiove oil platform in the North Sea. Of the 37 men who survived, and of the safety boat's crew, we interview a handful that draw the terrible conclusion that the tragedy could have been avoided: design flaws and faulty safety protocols led to a towering inferno in the middle of the freezing sea.
